I understand: 1. This transfer of annual leave may result in a reduction in an individual's salary as the agency calculates it. For example, if a senior employee had scheduled 5 days during his 10-year work history, transfer to an approved leave recipient may result in the agency reducing the employee's salary to 4 days. 2. As of the date indicated below, I have enough annual leave in my account to cover this amount. I also understand that the amount of annual leave I am transferring may not exceed the number of hours remaining in the leave year for which I am scheduled to work. I understand that if I am projected to earn less money in the future than my prior earned leave balance I may request leave of absence not covered by this transfer if the agency believes it would be feasible. Please provide the following information: If you are requesting leave to which an annual leave account is designated, please indicate at the top of the “Request for Transfer” document which employee, if related to the request, is designated as the approved leave recipient when the annual leave transfer is requested.
If you are requesting transfer to a leave recipient with approval from your immediate supervisor, please indicate the employee's name at the top of the “Request for Transfer” document. This statement does not need to include supervisor name.
If you are requesting transfer to leave recipient with approval from the employee(s) receiving the leave, please indicate the employee's name at the top of the “Request for Transfer” document. This statement does not need to include supervisor name.
The employee requesting the transfer is the primary beneficiary of the annual leave account (not a designated beneficiary).
For each employee requesting an annual leave transfer, please attach a copy of the employee's current and last-year leave and/or salary schedules. In addition, you should attach the statement from the approved leave recipient indicating which employee or employees will be receiving annual leave in lieu of transfer.
If your request for transfer is approved, you will receive an electronic acknowledgement of the completion of the transfer request process and an email confirmation of approval. The employee receiving annual leave in lieu of transfer will be scheduled to work the same number of hours at the existing annual leave rate and rate of pay, as outlined in the employee's current pay schedule. If your request is not approved, your authorized supervisor will determine whether to accept your resignation or to transfer your salary or benefits to another employee. The “Request for Transfer” document will not be mailed to the employee.
